Job summary

Northwest Nursing and Rehab Center in Baltimore, MD is seeking compassionate Licensed Practical Nurses (LPNs) to provide high-quality nursing care to our residents. This role involves 12-hour shifts and collaboration with RNs to implement individualized care plans.

Responsibilities include administering medications, monitoring resident health, documenting care, and supporting a resident-centered care approach. CPR/BLS certification and MD LPN license required.

Qualifications

• Active LPN license in Maryland.
• Graduate of an accredited nursing program.
• Current CPR/BLS certification.
• Strong communication and critical thinking skills.
• Ability to work flexible schedules, including weekends and holidays if required.

Responsibilities

• Administering medications and treatments as ordered by physicians
• Monitor resident health and report changes to the care team
• Collaborate with RNs and care teams to develop and implement individualized care plans
• Maintain accurate and detailed documentation of resident care
