Job summary

LifeStream Behavioral Center is seeking an LPN to provide high-quality nursing care in a pool-based, flexible staffing role. You will support the healthcare team in delivering behavioral health services and ensure patient safety and wellbeing.

You will administer medications, monitor patients, document care, and collaborate with RNs and physicians on individualized plans. Strong communication, EHR proficiency, and adaptability are essential.

Qualifications

• Completion of an accredited LPN program.
• Current LPN license in the state of practice.
• 0–3 years of experience as an LPN; behavioral health preferred.
• Pool/flexible staffing experience desirable.
• Strong patient care, communication, and collaboration skills.

Responsibilities

• Administer medications and treatments as prescribed and per protocols.
• Monitor vital signs and patient status; report changes to the team.
• Collaborate with RNs, physicians, and other professionals on care plans.
• Assist admissions, transfers, and discharges with proper documentation.
• Provide patient education and documentation of care provided.
• Maintain a safe, clean environment and comply with regulations.

Job Purpose:- The Licensed Practical Nurse (LPN) at LifeStream Behavioral Center is responsible for providing high-quality nursing care to patients within the facility. This role involves working in a pool capacity, requiring flexibility and adaptability to meet the varying needs of the center.
