Responsibilities

• Provide patient care under supervision, following LPN scope and clinical guidelines.
• Document patient status and care plans accurately in electronic health records.
• Monitor vital signs and report variances to nursing team promptly.
• Assist with medication administration and track dosage accuracy.
• Identify and elevate potential patient safety risks in a timely manner.
• Collaborate with physicians and RNs to adjust care plans based on progress.
• Maintain a clean, safe, and compliant clinical environment.
• Engage in continuous learning to improve clinical skills and clinical decision-making.
• Support patient and family education on care plans and post-discharge steps.
• Participate in on-site shift handoffs and ensure continuity of care.
• Assist in routine clinical tasks to improve operational efficiency.
• Contribute to performance metrics by meeting individual and team targets.

Experience/Qualifications

• Licensed Practical Nurse (LPN) license valid in applicable state.
• Proficiency with electronic health records and care plan documentation.
• Vital signs monitoring with rapid escalation of variances.
• Medication administration support with dosage accuracy and safety checks.
• Strong patient safety mindset and escalation of potential risks.
• Ability to collaborate with physicians and RNs to adjust care plans.
• Maintain clean, safe, and compliant clinical environment per policies.
