Job summary

Trinity Health System is seeking a Licensed Practical Nurse to join our hospital team. The role involves providing nursing care under the supervision of the Registered Nurse and collaborating with an interdisciplinary team to support patient needs.

Responsibilities include data collection for care assessment, planning and implementing patient care activities, patient education, and timely communication about changes in health status.

• Contributes to the assessment of patient care through the collection and reporting of objective and subjective data.
• Assists in the planning of patient care through participation in interdisciplinary care.
• Assists with implementation of patient care activities, including assistance with patient education and medication administration to meet the health care needs of the patient.
• Assists with the evaluation of patient care through the prompt communication regarding changes in the healthcare needs of the patient.
• Maintains the competencies necessary to execute the skills necessary to fulfill the duties of the position.
• Perform other duties as assigned.

Job description
