Job description

Job Summary

The Licensed Practical Nurse (LPN) will assess and evaluate patients' physical, psychosocial, environmental, spiritual, social, and educational needs. They will document responses to nursing interventions and collaborate with a multidisciplinary team to achieve optimal patient care outcomes.

Key Responsibilities
  1. Assist in developing and implementing patient care plans, including discharge planning, based on nursing standards, policies, and patient needs, utilizing critical thinking skills.
  2. Provide and facilitate educational opportunities tailored to patient and family needs, based on the nursing care plan.
  3. Adapt positively to changes in the healthcare environment, supporting new technology and processes, maintaining a calm and positive demeanor under stress.
  4. Communicate effectively regarding patient status changes to registered nurses and other healthcare team members.
  5. Participate in departmental meetings, in-services, and educational activities with at least 80% attendance, including 50% in-person if available via webinar.
  6. Manage resources efficiently, support staffing needs across units, and meet productivity expectations.
  7. Contribute to process improvement initiatives to enhance patient satisfaction, clinical outcomes, and financial performance.
  8. Accurately document patient information in medical records in a timely manner.
  9. Delegate and accept delegation appropriately, based on patient needs and scope of practice.
  10. Identify personal learning needs and participate in professional development to meet annual goals.
  11. Maintain confidentiality and comply with HIPAA regulations.
  12. Provide competent patient care, interpreting data and adapting procedures based on age-specific needs.
  13. Engage in ongoing personal and professional growth through departmental and center-wide educational programs.
  14. Uphold the organization's mission, values, and cultural beliefs in all duties.
  15. Support safety policies and proactively address safety concerns.
  16. Perform additional duties as assigned by supervisors.
