Job summary

Vernon Health and Living in Wabash, IN is seeking a dedicated Licensed Practical Nurse to provide direct, supervised nursing care to residents. You will administer medications, monitor conditions, and support the health and comfort of our residents in a warm, family-like team environment.

You will work under an RN, perform wound, catheter, ostomy care, take vital signs, and help implement care plans. Successful candidates have LPN licensure and current BLS/CPR, with LTC experience preferred.

Qualifications

• Current, unrestricted LPN license in the state of practice.
• Graduate of an accredited practical nursing program.
• Current BLS/CPR certification.
• Long-term care or geriatric nursing experience preferred.
• Strong medication administration and observation skills.
• Ability to work rotating shifts, weekends, and holidays.

Responsibilities

• Administer medications per physician orders.
• Monitor residents for changes and report to RN.
• Perform wound, catheter, ostomy care and other treatments.
• Obtain and document vital signs, weights, and intake/outtake.
• Implement care plans as directed by the RN and care team.
• Supervise and delegate tasks to CNAs on your shift.
• Communicate with residents, families, and the nursing team.
• Document care accurately and timely in the electronic health record.
• Follow infection control, safety, and facility standards.
