Akkermans Leisure & Golf is seeking a dedicated Licensed Practical Nurse to lead health and wellbeing for residents in a full‑time setting. The role involves 12‑hour overnight and day shifts, supervising CNAs and coordinating with a pharmacy consultant to ensure safe medication administration.

Ideal candidates will have an active LPN license, 2+ years in assisted living or long‑term care, and strong assessment, triage, and communication skills to maintain high-quality resident care and safety

• Maintains current state LPN license per state regulations.
• Experience in assisted living, home health, or long-term care industries.
• Two years experience as an LPN.
• Competence in assessment, injections/medication administration, follow-up, charting, triage, communication, and team leading/supervision.

Responsabilidades

• Ensures community follows all federal, state, local laws and regulations as it relates to clinical care.
• Collects and reports occurrences; assists in improvement plans as needed.
• Partners with Administrator and team to maintain resident and team health safety.
• Partners with pharmacy consultant to provide optimal pharmaceutical services to residents.
• Responsible for infection control practices in community.
• Oversees safe/distribution/administration of medications and injections when floor nurse unavailable.
• Identifies resident health needs through evaluation/service plan process and documents changes.

Position Summary

Responsible for providing overall leadership and management of the health and wellbeing of the residents within the community.

SHIFT:

FULL TIME Overnight and Dayshifts (12-hour shifts)

Essential Duties And Responsibilities

The following duties are normal for this position. This list is not to be construed as exclusive or all inclusive. Other duties may be required and assigned.

• Maintains communications with Healthcare Providers of recent changes or requests for orders.
• Ensures that community follows all federal, state, local laws and regulations as it pertains to clinical care.
• Responsible for collecting, and reporting occurrences, and if necessary, assist in developing/implementing improvement plans.
• Partners with Administrator and other team members to maintain resident and team member health safety.
• Partners with pharmacy consultant to provide optimal pharmaceutical services to residents.
• Responsible for infection control practices in community.
• Responsible for the safe and efficient distribution/administration of medications, injections, nebulizers, moderate wound care per state regulations, as needed when floor nurse is not available.
• Identifies ongoing needs and services of residents through the evaluation/ Service Plan process including documentation for residents with a change in health care status.
• Responsible for the completion of any assigned evaluations.
• Partners with other department coordinators to identify, review, and discuss potential change in service/medication levels and updates the resident’s service plan as indicated by state regulations.
• Maintains communications with resident’s family and/or responsible party regarding changes in care or health concerns.
• Other duties as assigned.

Supervisory Responsibilities

Provides supervision to Certified Nursing Assistants and all direct care staff as assigned to assure delivery of consistently high quality care to residents. Carries out supervisory responsibilities in accordance with the organization's policies and applicable laws.

Educational Requirements And Experience

• Maintains current state license as a Licensed Practical Nurse per state regulations.
• Experience in assisted living, home health, or long-term care industries.
• Two (2) years experience as a LPN.
• Must demonstrate competence in assessment skills, injections/medication administration, follow up, charting, triage, communication, and effective team leading/supervision.
