Horizon House, Inc. in Kentucky's Northern region seeks a Full-Time Licensed Practical Nurse (LPN) to coordinate healthcare needs for participants with behavioral health concerns.

The role uses the nursing process for planning, implementing, and educating residents and families to support independence. You will work with psychiatrists and clinic staff, assist in the Clozaril Clinic, and help ensure lab tests are ordered and results reviewed by the psychiatrist, while maintaining strong teamwork

Qualifications

• Licensed Practical Nurse (LPN) with certification.
• Experience in community nursing and mental health with co-occurring disorders.
• Experience serving psychiatric needs in mental health.
• Strong interpersonal skills, organization, and analytical skills.
• Available for emergency on-call service.
• Valid driver’s license.
• Ability to work independently and as part of a focused team.

Responsibilities

Job description

We are seeking qualified candidates for the Full-Time Licensed Practical Nurse (LPN) position. To coordinate the healthcare needs of the person with behavioral healthcare concerns. Deliver care to participants utilizing the nursing process, including the planning of care, implementation, and resident and family education. Instruct, counsel, and support assigned participants, focusing on their independence and individual needs. Maintain an environment that encourages proactive decision-making and growth for each participant.

Other Responsibilities, but not all-inclusive

• To provide and assist psychiatrists and team members in coordinating the behavioral and physical health care needs of program participants.
• Assist in the Clozaril Clinic to ensure efficient flow of information from and to the psychiatrists. Work in conjunction with other nurses to complete nursing duties and documentation in the clinic.
• Develop and monitor systems to ensure participants have prescribed laboratory tests and that results are received and reviewed by the ordering psychiatrist.

Qualifications

• Completion of a certified nursing course as evidenced by a license as a Licensed Practical Nurse (L.P.N.). Experience in community nursing and mental health with co-occurring disorders.
• Prior experience in serving psychiatric needs in mental health
• Comprehensive knowledge of psychiatric clinical and psychosocial rehabilitation practices
• Strong interpersonal skills, organization, and analytical skills.
• Must be available for emergency on-call service.
• Excellent written and verbal communication skills
• Valid driver's license required
• Ability to work independently and as part of a focused team

Scope

• Ability to lift participants of various weights while maintaining participants' comfort and safety.
• Frequent talking and listening.
• Approximately 75% of the work is in the community.
• May require crisis intervention in emergencies.
• May be required to report to work during emergencies, including inclement weather
