Job summary

Horizon House, Inc. in Sussex, DE seeks a Licensed Practical Nurse (LPN) to join its Residential Group Home team. You will participate in recovery planning, coordinate with staff and families, and monitor participants’ mental health and medication administration.

Qualified candidates hold a Delaware LPN license or are in the Nursing Licensure Compact including DE, with strong communication and independent work skills. Local travel may be required; on-call work possible.

Qualifications

• LPN license in good standing in Delaware or eligible via Nursing Licensure Compact (DE included).
• Knowledge of mental health systems and supports.
• Knowledge of substance use treatment and supports.
• Strong communication skills, both oral and written.
• Ability to work independently and manage a caseload.
• Valid driver’s license with clean driving record.
• Willingness to undergo background checks per state requirements.
• Flexibility in scheduling and potential on-call duties.

Responsibilities

• Assist in development and evaluation of recovery plans with participants and staff.

Job description

Horizon House is a nationally recognized provider of services to adults with support needs due to developmental disabilities, behavioral health issues, and substance abuse. Horizon House is Pennsylvania’s oldest psychiatric rehabilitation agency. Since 1952, Horizon House has been dedicated to helping people with mental illness, drug and alcohol addictions, developmental/intellectual/disabilities, and homelessness to help stabilize their lives and to help them live as independently as possible in the community. With over 100 locations throughout Southwestern Pennsylvania and the state of Delaware, Horizon House provides services to a diverse population, focusing on individual strength and choice.

We are seeking qualified Licensed Practical Nurse (LPN) interested in joining ourResidential Group Hometeam at our Sussex location.

Some of the responsibilities include the following.

Assist in the development, implementation, evaluation, and revision of recovery plans in collaboration with the participants and assigned staff.

Maintains a case load of assigned participants and completes assigned paperwork in a timely manner.

Act as a liaison, consulting with community agencies and families to maintain coordination of care in the treatment process.

Provides ongoing assessment of participants’ mental health symptoms and responds to recovery plans.

Educates and supervises participants in self-administration of medication.

Ensures annual review of the participants physical condition and assist access to and encourage compliance with treatment for identified physical problems.

Qualifications:

Board Certified Licensed LPN in the state of Delaware; or participating in the Nursing Licensure Compact to include the state of DE.

Knowledge of mental health systems and supports.

Knowledge of substance abuse systems and supports.

Good communication skills both oral and written.

Ability to work independently.

Possess a valid driver’s license with an acceptable driving record.

Strong organizational skills.

Successful completion of the Child Abuse Registry and the Adult Abuse Registry background check with no adverse registry findings, and obtain service letters in accordance with 19Del Code Section 708 and 11 Del Code Sections 8563 and 8564.

Scope:

Frequent talking and listening.

Requires the ability to lift, carry, hold, kneel, stack, stoop, reach and bend which may include direct intervention with participants.

Local travel required, including transporting participants.

May be required to report to work during emergencies, including inclement weather.

May require crisis intervention in emergency situations.

Scheduling flexibility and possible on-call work are necessary.
