Job summary

Montefiore Medical Center in the Bronx is seeking a Licensed Practical Nurse (LPN) to provide direct clinical care for a patient population within the Psychiatry - SATP department. The role requires NYS LPN licensure, BLS certification, and strong communication skills.

You will work day shifts with alternate weekends, 10:30 AM–7 PM, at an hourly rate of $38.99. Duties include patient education, discharge planning, documentation, and contributing to quality improvement while maintaining

Qualifications

• High School Diploma or GED required; Associate degree preferred.
• New York State Licensed Practical Nurse (LPN) license required
• Basic Life Support (BLS) certification required, strong English communication and interpersonal skills

Responsibilities

• Provide direct care for a population-specific patient assignment reflecting current status.
• Provide patient/family teaching and discharge instructions to meet learning needs.
• Set priorities, problem-solve, and use proper judgment in difficult situations.
• Document patient care per policies and procedures
• Participate in continuous quality improvement and data collection relevant to planning/evaluation of patient care
• Communicate effectively with patients/families and health care team members
• Demonstrate medication administration as per MMC policy and procedures

He/she is committed to maintaining the standards of the practice of nursing and is responsible for the delivery of care to the neonate, pediatric, adolescent, adult and/or elderly patient within their scope of practice.
The LPN participates in performance improvement, research activities and maintains clinical expertise

Essential Functions

• Provide direct care for a population specific patient assignment reflective of current and status of patient.
• Provide patient / family teaching and discharge instructions to meet learning/ discharge needs as identified by RN.
• Set priorities, problem-solve, and use proper judgment in difficult situations and be flexible in non-emergent situations
• Provide and maintain safe environment for patients/families through preventative and interventional measures
• Document patient care per policies and procedures
• Participate, promote, and practice continuous quality improvement. Collect and document of data relevant to planning / evaluation of patient care
• Communicate effectively with patients/families and members of the health care team
• Demonstrate medication administration as per MMC policy and procedure
• Responsible and accountable for own practice
• Demonstrate body mechanics as it relates to lifting / transfer / procedural assistance and knowledge and skills necessary to provide care appropriate to patient population served
• Use Blood pressure, temperature, breathalyzer, pulse oximeter equipment, electronic clinical information system, and perform phlebotomy
• Responsible for maintenance of Customer Service
• Demonstrate knowledge and skills necessary to provide care appropriate to the age of the patient’s served
• Utilize a teaching and discharge plan based on documented patient/family learning needs, chronological maturation and cognitive ability

Qualifications

• High School Diploma / GED required, Associate’s Degree preferred
• New York State License for Practical Nurse / LPN & Basic Life Support Certification are required
• Effective English communication skills, written, oral and interpersonal required
• Ability to use basic data entry and retrieval system required
• Additional education required by specialty areas/patient care locations
