Qualifications

• Current LPN license in New Jersey.
• HS diploma or equivalent required; nursing training a plus.
• Prior experience in long-term care, rehabilitation, or post-acute care is a plus.
• Ability to lift, push, or pull up to 50 pounds regularly.

Responsibilities

• Provide nursing care to residents per individualized treatment plans.
• Administer medications, injections, and treatments as prescribed by physicians.
• Perform wound care, phlebotomy, and EKGs.
• Monitor and report changes in resident condition to the healthcare team.
• Collaborate with physicians, therapists and other professionals to develop care plans.
• Document resident care and progress in electronic medical records.
• Maintain a clean and safe environment for residents and staff.
• Participate in educational in-services and training programs.

Responsibilities

• Provide nursing care to residents in accordance with their individualized treatment plans
