Job summary

Platte Health Center Avera in Platte, SD seeks a qualified nurse (LPN or RN) to supervise and provide patient care using the nursing process. The role emphasizes teamwork, accountability, and age-appropriate collaborative care.

Licensed in SD is required; BLS must be obtained within six months of hire. Long Term Care experience is helpful but not required, with 3–5 shifts weekly and 9.5 or 12 hour shifts available.

Qualifications

• Eligible for or currently licensed as an LPN or an RN in the state of South Dakota.
• Basic Life Support is mandatory within six months of hire.
• Previous experience in Long Term Care is helpful, but not required.

Responsibilities

• Supervises and provides care to patients.
• Actively uses the nursing process in resident centered patient care, demonstrating accountability as an individual and team member.
• Synthesizes information from a variety of sources to develop and implement an individualized, age appropriate, collaborative plan of care.
• Provides timely, continuous, age appropriate, collaborative assessment and preparation of patients and family.
• Assesses patients' and families' age appropriate learning needs, coordinates necessary teaching and evaluates teaching outcomes.
• Demonstrates knowledge of and utilizes safe patient care protocols in line with standards and unit policies.
• Documents in the medical record.
• Completes physician rounds as needed.
• Performs procedures with minimal supervision.
• Adheres to department policies and procedures.
