Job summary

HOMECARE WITH HEART, LLC is seeking a compassionate LPN to provide in-home nursing care in the Ohio area. The nurse will oversee quality of care, administer medications, and support patients in maintaining independence in their homes.

A valid Ohio LPN license and driver’s license are required. The role involves visiting client homes Monday through Friday, with a focus on safety, care coordination, and documentation.

Qualifications

• Valid State LPN License.
• Valid driver’s license and access to a vehicle.
• Annual MUI training as required by Ohio Medicaid and related programs.
• Ability to read and understand Plan of Care, policies, and procedures.
• Must be able to use a computer, laptop or tablet.
• Must be able to think independently and report changes to a supervisor.

We're looking for compassionate and experienced nurses to provide care for our clients in their homes. As a Home Care Nurse, you will be responsible for overseeing the quality, expert care of your assigned homecare patients. Pay for LPN: $27 per hour depending on experience and availability.

Responsibilities

What You’ll Do:

• Administer medication in accordance with standards of practice.
• Encourage social ehabilitative skills and encourage self-care and independence.
• Perform client treatments in accordance with Plan of Care
• Assist the consumer in learning appropriate self-care techniques.
• Prepare clinical and progress notes to document care provided.
• Monitor, document accordingly and report any change in client condition to Clinical Director/Clinical Supervisor
• Promote activities to achieve a safe and healthy environment.
• Observe aseptic technique when performing client procedures/care.
• Collaborate with other members of the clinical team to develop and improve the Plan of Care
• Assist the client in achieving established goals in the Plan of Care developed by the RN.
• Maintain productivity according to the standards of the agency.
• Maintain license, CEUs and mandatory employment requirements.
• Participate in client care meetings, in-services, orientation and staff meetings.
• Provide service in accordance with agency’s philosophy, standards, policies and scope of practice Participates in Performance Improvement and adheres to HIPAA guidelines.
• Other duties as assigned.

• Annual MUI (Incident) training as dictated by the State of Ohio Medicaid, Ohio Home Care Program and Department of Developmental Disabilities
• Must be able to read and understand various documents used by the agency including Plan of Care, flow charts, client charts, agency policies and procedures.
• Must be able to use a computer, laptop or tablet.
• Must be able to think independently, recognize potential problems, gather information, and know when to contact a supervisor.
