Job summary

Homecare with Heart is seeking a compassionate LPN to provide in-home care for clients in Ravenna, OH. The starting rate is $27 per hour, with a $1,500 sign-on bonus and an 8p–8a schedule (every other Friday, Saturday, Sunday). A shift differential is offered.

Responsibilities include medication administration, applying Plan of Care, documenting progress, and collaborating with the clinical team to ensure high-quality patient care. Valid Ohio LPN license and reliable transportation required.

Qualifications

• Valid LPN license required.
• Must be able to read and understand Plan of Care, flow charts, client charts, agency policies and procedures.
• Must be able to use a computer, laptop or tablet.

Responsibilities

• Encourage social rehabilitation skills and self-care and independence.
• Perform client treatments per Plan of Care.
• Assist client in learning self-care techniques.
• Prepare clinical and progress notes to document care provided.
• Monitor and report any change in client condition to Clinical Director/Supervisor.
• Promote activities for a safe and healthy environment.
• Observe aseptic technique during client procedures/care.
• Collaborate with the clinical team to improve the Plan of Care.
• Assist client in achieving goals in the Plan of Care developed by the RN.
• Maintain productivity per agency standards.
• Maintain license, CEUs and employment requirements.
• Participate in client care meetings and staff trainings.
• Provide service per agency philosophy, standards and policies.

Homecare with Heart is offering you the most competitive pay scale in the area. We're looking for compassionate and experienced nurses to provide care for our clients in their homes. As a Home Care Nurse, you will be responsible for overseeing the quality, expert care of your assigned homecare patients. Starting rate for LPN: $27 per hour depending on experienceand availability.

Responsibilities

What You’ll Do:

• Administer medication in accordance with standards of practice.


• Encourage social\\rehabilitative skills and encourage self-care and independence.


• Perform client treatments in accordance with Plan of Care


• Assist the consumer in learning appropriate self-care techniques.


• Prepare clinical and progress notes to document care provided.


• Monitor, document accordingly and report any change in client condition to Clinical Director/Clinical Supervisor


• Promote activities to achieve a safe and healthy environment.


• Observe aseptic technique when performing client procedures/care.


• Collaborate with other members of the clinical team to develop and improve the Plan of Care

• Participates in Performance Improvement and adheres to HIPAA guidelines.

What You’ll Bring to the Team:

• Valid State LPN License


• Valid driver’s license and use of automobile is required due to the travel involved.


• Annual MUI (Incident) training as dictated by the State of Ohio Medicaid, Ohio Home Care Program and Department of Developmental Disabilities


• Must be able to read and understand various documents used by the agency including Plan of Care, flow charts, client charts, agency policies and procedures.


• Must be able to use a computer, laptop or tablet.


• Must be able to think independently, recognize potential problems, gather information, and know when to contact a supervisor.
