Job summary

CareRing Health, LLC is seeking an experienced LPN for a Monday-Friday 5p-9p homecare role. You will oversee quality, expert care for homebound clients and document progress. The position offers $27 per hour depending on experience and availability, plus a $1,500 sign-on bonus.

Join us to support clients in their homes through compassionate, patient-centered care and collaborate with the clinical team to optimize the Plan of Care.

Qualifications

• Must be able to read and understand various documents used by the agency including Plan of Care, flow charts, client charts, agency policies and procedures.
• Must be able to use a computer, laptop or tablet.
• Must be able to think independently, recognize potential problems, gather information, and know when to contact a supervisor.

Responsibilities

• Administer medication in accordance with standards of practice.
• Encourage social and rehabilitative skills and encourage self-care and independence.
• Perform client treatments in accordance with Plan of Care.
• Assist the consumer in learning appropriate self-care techniques.
• Prepare clinical and progress notes to document care provided.
• Monitor, document accordingly and report any change in client condition to Clinical Director/Clinical Supervisor.
• Promote activities to achieve a safe and healthy environment.
• Observe aseptic technique when performing client procedures/care.
• Collaborate with other members of the clinical team to develop and improve the Plan of Care.
• Assist the client in achieving established goals in the Plan of Care developed by the RN.
• Maintain productivity according to the standards of the agency.
• Maintain license, CEUs and mandatory employment requirements.
• Participate in client care meetings, in-services, orientation and staff meetings.
• Provide service in accordance with agency’s philosophy, standards, policies and scope of practice Participates in Performance Improvement and adheres to HIPAA guidelines.
• Other duties as assigned.

Qualifications

What You’ll Bring to the Team:

• Valid State LPN License
• Valid driver’s license and use of automobile is required due to the travel involved.

• Annual MUI (Incident) training as dictated by the State of Ohio Medicaid, Ohio Home Care Program and Department of Developmental Disabilities
• Must be able to read and understand various documents used by the agency including Plan of Care, flow charts, client charts, agency policies and procedures.
• Must be able to use a computer, laptop or tablet.
