Qualifications

• Licensed Practical Nurse (LPN) licensed in New Jersey.
• Experience in skilled nursing or long-term care preferred.
• Strong patient assessment and communication skills.

Responsibilities

• Administer medications and treatments as prescribed by physicians.
• Monitor and record vital signs, including blood pressure, temperature, and pulse.
• Develop and implement individualized care plans, in collaboration with healthcare teams.
• Provide wound care and document wound care accurately.
• Assist with activities of daily living such as bathing, dressing and grooming.
• Participate in quality improvement initiatives and maintain records.
• Collaborate with other healthcare professionals to ensure comprehensive care.
