Responsibilities

• Deliver telehealth services to patients, ensuring accessibility and high-quality care in virtual settings.
• Administer, score, and interpret validated behavioral health screening tools (e.g., PHQ-9, GAD-7) to identify patient needs.
• Document screening outcomes and provide feedback to patients and the care team.
• Develop action plans based on screening results and ensure follow-ups.
• Act as a patient advocate to ensure timely access to appropriate behavioral health services.
• Educate patients and families about behavioral health conditions coping strategies, treatment plans, and available resources.
• Provide case management services to ensure continuity of care, including referrals to community resources and specialized services.
• Monitor and document patient progress, modifying care plans as needed.
• Assist in the development and implementation of behavioral health programs to meet patient and community needs.

The LPCC/LISW supports the delivery of high-quality behavioral health services through counseling, care coordination, and collaboration. This role includes overseeing behavioral health assessments, providing telehealth support, and implementing behavioral health screening tools to optimize patient care outcomes.
