Job summary

Wellness Pointe is seeking a Counselor to provide behavioral health services within a multifaceted clinic in Texas. You will support emotional and mental health growth, work with a primary care team, and ensure adherence to agency guidelines and FQHC standards.

The role requires a Master's in Clinical Counseling and a Texas LPC or LCSW license. English-Spanish bilingual abilities are preferred. A sign-on bonus is offered and Medicaid/Medicare participation is supported.

Qualifications

• Master's degree in Clinical Counseling is required.
• Current Texas LPC License or LCSW license is required.
• Knowledge of mental health assessment, treatment planning, DSM diagnoses, and behavioral therapy.

Responsibilities

• Provide guidance and instruction to foster emotional, physical and mental growth of clients through therapeutic intervention.
• Assist clients in accordance with Agency and FQHC guidelines and meet productivity targets for billable services.
• Conduct Patient intakes with diagnostic evaluations and formulate treatment goals and plans.
• Provide brief, focused interventions for patients in need of mental health services.
• Advise on referrals to specialty mental health facilities when appropriate.
• Initiate follow-up to assess progress and adjust treatment plans as needed.
• Develop relapse prevention plans and help patients maintain stable functioning.
• Coordinate care with primary care providers and other treatment providers with patient consent.
• Document patient progress and diagnostic information in treatment records.
• Keep PCPs informed and collaborate to formulate treatment plans.
