Job summary

Loyola Medicine Home Care and Hospice is seeking a Physical Therapist to provide one-to-one, patient-centered care in patients' homes across the Oak Brook area.

You'll collaborate with physicians, nursing staff, social workers, and home health aides, using cutting-edge technology and Home Care Connect to support clinical excellence and patient goals. A valid IL PT license and reliable transportation are required.

Qualifications

• Bachelor’s degree with progressive experience or Master’s degree.
• Licensed to practice as a physical therapist in Illinois.
• Current driver’s license and reliable transportation to and from work site.
• Knowledge of conditions of participation and home healthcare experience preferred.

Responsibilities

• Provide one-to-one care with patients in their homes.
• Collaborate with physicians, nursing team, social workers, and home health aides.
• Utilize Home Care Connect and other technology to support patient care.
• Maintain organized, detail-oriented, and technologically proficient practice.

Physical Therapist position summary

Our home care physical therapists use cutting edge technology and clinical knowledge to provide exceptional care to patients who require one-to-one attention and monitoring in their home. Compassionate care is provided to all members of our communities through a multidisciplinary approach working collaboratively with physicians, nursing team members, social workers, and home health aides. Enthusiastic candidates must excel in an autonomous work environment, be highly motivated, detail orientated, organized and technologically proficient.
