Job summary

ADP, Inc. is seeking a Long-Term Care Pharmacist to lead clinical care and medication management for facility residents in a fast-paced setting.

The role includes drug regimen reviews, dosage tailoring for geriatrics, and monitoring high‑risk medications, with involvement in care plans alongside physicians and nursing staff. Qualifications

• Pharm.D. or B.S. Pharm from an accredited institution.
• Active Pharmacist License in good standing.
• 1–3 years of pharmacy experience in LTC, hospital/acute care, or mail-order settings preferred.
• Board Certified Geriatric Pharmacist (BCGP) or BCPS preferred.

Responsibilities

• Perform comprehensive drug regimen reviews for facility residents to prevent adverse drug events and interactions.
• Evaluate charts, lab results, and renal function to tailor dosages for geriatric populations.
• Monitor high-risk meds and recommend dose reductions or tapers.
• Participate in care plan meetings with physicians, nursing staff, and families.
• Review provider orders for accuracy and regulatory compliance.
• Oversee LTC packaging and supervise pharmacy staff.
• Manage emergency drug kits and automated dispensing cabinets at facilities.
• Conduct monthly audits to ensure CMS and state regulations are met.

Long-Term Care Pharmacist

Key Responsibilities Clinical Care & Medication Management

• Perform comprehensive drug regimen reviews (DRR) for facility residents to prevent adverse drug events, interactions, and therapeutic duplication.
• Evaluate patient charts, lab results, and renal function to tailor dosages for geriatric and specialized populations.
• Monitor high-risk medications (e.g., psychotropics, anticoagulants, antibiotics) and recommend appropriate dose reductions or tapers.
• Participate in care plan meetings with physicians, nursing staff, and patient families.

Dispensing & Operations

• Review and verify provider medication orders for accuracy and regulatory compliance.
• Oversee specialized LTC packaging (e.g., blister packs, unit-dose systems, multi-dose strip packaging).
• Direct and supervise pharmacy technicians, compounding staff, and fulfillment personnel.
• Manage emergency drug kits (E-kits) and automated dispensing cabinets housed at long-term care facilities.

Consulting & Regulatory Compliance

• Conduct monthly on-site or remote facility audits to ensure federal (CMS) and state long-term care regulations are met.
• Assist facilities in developing medication administration, storage, and disposal policies.
• Track and report quality metrics, medication errors, and antibiotic stewardship indicators.

Qualifications & Requirements

• Education: Doctor of Pharmacy (Pharm.D.) or Bachelor of Science in Pharmacy (B.S. Pharm) from an accredited institution.
• Licensure: Active Pharmacist License in good standing in the state of practice.
• Experience: 1-3 years of pharmacy experience in LTC, hospital/acute care, or mail-order environments preferred.
• Certifications (Preferred): Board Certified Geriatric Pharmacist (BCGP) or Board Certified Pharmacotherapy Specialist (BCPS).
• Extensive knowledge of geriatric pharmacotherapy, pharmacokinetics, and chronic disease management.
• Strong familiarity with CMS guidelines (e.g., F-Tags related to pharmacy services in nursing facilities).
• Proficiency with pharmacy management software and Electronic Health Record (EHR) integration.
• High attention to detail, precision, and problem-solving skills in high-volume settings.
