Job summary

Harbor Healthcare System is seeking a full-time Long Term Care Liaison LVN in the Clear Lake area. You will work closely with the clinical team, patients, families and nursing facilities to coordinate care and ensure quality outcomes.

The ideal candidate will be a Texas-licensed LVN with experience in long-term care, strong communication skills, and the ability to engage effectively with diverse stakeholders. Public speaking and reliable transportation are expected.

Qualifications

• Must have strong oral and written communication skills.
• Experience with long-term care is required.
• Public speaking ability is preferred.
• Prior experience in hospice or home health care is advantageous.
• Must hold a valid Texas LVN license.

Job Description:

The Long Term Care Liaison LVN positionsworks closely with the Clinical team, patients, families and nursing facilities.

Qualifications:

• Graduate of an accredited college or university or two years of experience working in hospice and/or home health patient care
• Must be a Licensed Vocational Nurse (LVN) in the State of Texas
• Experience with long term care - required
• Must have good oral communication and writing skills
• Must be able to speak well in public
• Must be able to practice the principles of good public relations
• Must have a current state driver's license
• Must have reliable transportation
• Must have a current auto insurance policy
• Clean background check and drug screen
