Healthfirst is seeking a Care Coordinator to perform care management services for NorthStar Members, supporting multiple teams to deliver high-quality customer service. The role involves handling member calls, documenting interactions, and coordinating with care managers to identify risk factors requiring medical intervention.
The position requires CMA/CNA/RMA or equivalent training, HS diploma, and strong verbal/written communication in a fast-paced environment.
Care Coordinators are responsible for performing care management services in order to support members who require assistance in navigating their health care system. The Care Coordinator supports various teams to provide excellent customer service to our NorthStar Members.
As a candidate for this position, your salary and related elements of compensation will be contingent upon your work experience, education, licenses and certifications, and any other factors Healthfirst deems pertinent to the hiring decision.
