Healthfirst is seeking a Care Coordinator to support NorthStar Members by performing care management services and coordinating with interdisciplinary teams. You will handle incoming calls, document in the PHI system, and manage a caseload to help navigate care and improve outcomes.
The role requires CMA/CNA/RMA or medical training, HS diploma, and strong communication skills. Experience in fast-paced environments and call centers is a plus. Healthfirst offers benefits and salary ranges as posted.
Care Coordinators are responsible for performing care management services in order to support members who require assistance in navigating their health care system. The Care Coordinator supports various teams to provide excellent customer service to our NorthStar Members.
