Job summary

My Access Medical Clinic seeks a Long-Term Care Advanced Practitioner (APP) to serve as a dedicated clinical partner in skilled nursing and post-acute facilities. The APP provides autonomous clinical care, collaborates with medical directors, and supports care delivery, with emphasis on reducing hospital transfers and improving resident stability.

Qualifications

• Active license in applicable state and license in good standing.
• Board Certification (AANP/ANCC/NCCPA).
• DEA and BLS certifications.
• Excellent interpersonal communication skills.
• Ability to interpret and apply treatment protocol guidelines.

Responsibilities

• Perform comprehensive and ongoing clinical assessments for LTC residents.
• Diagnose and manage acute and chronic disease processes common to LTC populations.
• Order and interpret necessary diagnostic, laboratory and radiographic data.
• Develop, implement, and adjust individualized care plans in collaboration with facility teams.
• Provide advanced management of high-risk LTC clinical conditions including infections and sepsis risk mitigation, wound care, and chronic disease optimization.
• Function as a consistent clinical presence within assigned facilities and collaborate with leadership.
• Support readmission reduction and improving resident stability and transitions of care.
• Maintain accurate, timely, and compliant clinical documentation and licensure.
