Job summary

Personal Touch Home Health Care Services Inc. is seeking a PRN LTAC RN to provide high-acuity, long-term care at partner facilities. You will perform comprehensive patient assessments, develop individualized care plans, and manage ventilators and IV therapy.

You will collaborate with an interdisciplinary team to ensure quality outcomes, transition patients from ICU-level care when needed, and document all care and treatments in the EMR with accuracy and professionalism.
