Human Touch Home Health is seeking Occupational Therapists for full-time, part-time, and PRN positions to serve Germantown, Clarksburg, Damascus and surrounding Northern Montgomery County communities.

Choose your schedule and territory with focused coverage in Germantown, Clarksburg, Damascus, or combinations. Full benefits available for eligible full-time staff, including medical/dental/vision, PTO, holidays, and mileage reimbursement.

Qualifikationen

• Active Maryland Occupational Therapist License Required.
• Strong clinical, communication, and organizational skills.
• Ability to work independently in patients' homes.
• Home Health Experience preferred, but not required.
• WellSky experience is a plus.

Aufgaben

• Complete occupational therapy evaluations in patients' homes.
• Develop individualized plans of care.
• Help patients regain independence with activities of daily living.
• Provide therapeutic interventions to improve functional ability.
• Address bathing, dressing, grooming, toileting, meal preparation, and household tasks.
• Evaluate home safety and fall risks.
• Recommend and train patients on adaptive equipment.
• Educate patients, families, and caregivers.
• Monitor patient progress.
• Complete reassessments and discharge planning.
• Collaborate with the interdisciplinary care team.
• Complete timely documentation using WellSky.
