Occnet is seeking an LHC Coordinator to support local Homeless Coalition activities, attend meetings, and coordinate with agencies across the Balance of State. You will assist in strategic planning, communications, and HMIS-related tasks while ensuring confidentiality and proper documentation.
The role requires traveling extensively, working with diverse populations, and maintaining strong collaboration with service providers. A high school diploma is required; a bachelor’s degree is preferred.
Duties and Responsibilities:
Attend all BoS and LHC meetings either in person or via Zoom
Attend and participate in CE committee meetings
Assist Director with:
Development of committees with strategic planning
Updates for social media and website
Involvement in Coordinated Entry and HMIS
Training agencies in usage
Maintain records of user agreements, training certification, and confidentiality to share information by all agencies using the system
Assist with oversight of annual PIT Count to include training and planning
Assist with preparation of annual NOFO
Determine assistance needed and agency invitations for each LHC within the BoS
Coordinate all CE Point of Contact agencies with updates of information and changes
Other duties as assigned by the System Analysis Director.
Requires extensive travel across the Balance of State Continuum of Care (46 counties)
Minimum Education and Experience:
The LHC Coordinator will have a minimum of a High School Diploma with 2-3 years’ experience working with persons in poverty and/or housing assistance, Bachelor’s Degree with 2 years’ experience. Bachelor Degree preferred
Experience working with homeless and survivors of domestic abuse
Proficient in documentation and case files
Skills and Knowledge:
Ability to relate in a professional manner with and make connections with outside agencies for collaboration with OCYFS
