Pride Health seeks a local contract Physical Therapist for a Home Health assignment in Red Bud, IL. The role supports our client’s Home Health Program with a 17-week contract and a 40-hour work week, Monday–Friday from 8:00 AM to 4:30 PM, in Red Bud, IL and surrounding areas.

Travel-ready clinicians may extend; pay ranges from $60.00–$66.00 per hour (W-2) plus mileage reimbursement. Mileage rate is $0.51 per mile. Active PT license required and home health experience preferred.

Qualifications

• Active Physical Therapist license required.
• Minimum 1 year of home health experience.
• Familiarity with HomeCare HomeBase (HCHB) and OASIS is a plus.

Responsibilities

• Perform comprehensive physical therapy evaluations and ongoing reassessments to identify impairments, functional limitations, and safety risks.
• Develop, implement, and adjust individualized care plans to improve mobility, strength, balance, endurance, pain management, and functional independence.
• Deliver skilled interventions including therapeutic exercise, gait training, transfer training, balance activities, stair training, and home safety education.
• Educate patients and caregivers on home exercise programs, fall prevention, assistive device use, and energy conservation techniques.
• Complete accurate and timely documentation using EMR systems.
• Coordinate care with physicians and interdisciplinary team members, including nurses, occupational therapists, speech therapists, and home health aides.
• Communicate changes in patient condition, safety concerns, or barriers to progress promptly to the care team.
• Participate in case conferences, care coordination activities, and team collaboration.
• Ensure compliance with CMS regulations, infection control standards, and agency policies.
• Manage a mobile caseload efficiently while meeting productivity and documentation expectations.
• Adapt quickly to new environments, workflows, and patient populations as a travel clinician.

Requirements

• License(s): Active Physical Therapist license
• Years of experience: At least 1 year of home health experience and experience with HomeCare HomeBase (HCHB) and OASIS is a plus

Note: Travel contracts are also available if you reside more than 50 miles from the specified location - Travel Pay Package: Up to $2,906.80/week gross (includes weekly non-taxable stipend)* Plus Mileage reimbursement.
