Job summary

PruittHealth @ Home is seeking a Licensed Medical Social Worker LMSW for Home Health Services in South Carolina. The role is per diem and involves delivering social work services at patients' homes within Horry County.

Qualifications

• Master's degree in social work from an accredited school of social work.
• One year of experience in medical social work.
• Valid and unrestricted professional license in state of practice.
• Valid driver's license and automobile liability insurance.
• Current CPR certification.

Responsibilities

• Provide medical social services to Home Health patients and families in their home.
• Complete psychosocial history and assessment to evaluate social service needs.
• Develop an initial plan of care with the RN and interdisciplinary team.
• Identify resources for financial, social, medical and emotional well-being.
• Counsel patients/caregivers to assist in coping with changes in condition.
• Maintain clinical records to meet regulations and guidelines.
• Report patient status and changes to the Interdisciplinary team.
• Document services and interactions in the medical record.
• Assist in long-term planning and discharge planning.
