Job summary

PruittHealth Corporation is seeking a Licensed Medical Social Worker LMSW for Home Health Services in Bluffton, SC. You will provide medical social services to patients in their homes and collaborate with an interdisciplinary team to plan care and discharge strategies.

Qualifications

• Master’s degree in social work from an accredited school.
• Minimum 1 year of experience in medical social work.
• Valid unrestricted professional license in state of practice.
• Valid driver’s license and automobile liability insurance.

Responsibilities

• Provide care to patients within the accepted standards of practice.
• Complete psychosocial history and assessment to evaluate social service needs.
• Develop an initial plan of care in consultation with the interdisciplinary team.
• Locate resources for the patient’s financial, social, medical and emotional well‑being.
• Provide counseling to patient/caregivers to assist in coping with changes.
• Maintain clinical records to meet regulations and guidelines.
• Inform the team of patient condition, outcomes and changes in needs.
• Complete and submit required documentation within guidelines.
• Document all services and interactions in the medical record.
• Assist in long-term planning and discharge planning.
• Be self-directed, manage time effectively, and meet deadlines.
