Job summary

Amedisys is seeking a PRN Medical Social Worker for day shifts in South Carolina. SC licensure is required to apply. This role supports patients and families at home within one of the nation’s leading home health and hospice companies.

Base pay is presented as a per-visit rate, with a comprehensive benefits package described in the posting. Responsibilities include social work services per care plans, patient/family education, coordinating with the care team, and risk assessment in the home

Qualifications

• Holds a masters or doctorate degree from a CSWE-accredited school.
• Current CPR certification.
• Meets state and federal requirements for this position.

Responsibilities

• Assists the patient and family with health-related financial, social and emotional concerns and helps with coping skills.
• Provides social work services per the plan of care.
• Communicates changes in clinical status to the health care team and care center staff in a timely manner.
• Educates patient and family on social work interventions and treatment plan.
• Performs care planning, transfer and discharge functions.
• Identifies risks in the patient’s environment and implements interventions to reduce risks.
• Other duties as assigned.

Education
