Job summary

Enhabit Home Health & Hospice seeks an experienced social worker with an MSW to provide patient-centered social services in the home health setting. You will educate patients and families, collaborate with the care team, and ensure services align with physician orders and the care plan.

Applicants must hold a state license, have at least one year of healthcare social work experience, and demonstrate basic tech skills (mobile devices). Medicare home health or hospice experience is preferred.

Qualifications

• Must have MSW from an accredited program.
• Must be licensed in the state where they practice.
• Must have at least one year of social work experience in a health care setting.
• Must have basic demonstrated technology skills, including operation of a mobile device.
• Medicare home health or hospice experience is preferred.

Responsibilities

• Administer social services to patients requiring intermittent professional services and teach the patient, family, and other members of the health care team.
• Perform services in accordance with the physician’s orders and the established plan of care (POC).
