Job summary

Bristol Hospice, LLC seeks a Licensed Master Social Worker (LMSW) for a full-time, on-site role in the Las Vegas Metropolitan Area. You will perform psychosocial assessments, provide counseling, and support patients and families navigating hospice care, grief, and end-of-life decisions.

Collaborating with an interdisciplinary team, you will develop individualized care plans, conduct home and facility visits, and coordinate community resources while documenting services and supporting bereavement

Qualifications

• MSW degree from an accredited program and LMSW licensure in the state.
• Demonstrated psychosocial assessment, counseling, and crisis intervention skills in medical, hospice, or palliative care.
• Experience collaborating with an interdisciplinary team to develop and implement care plans.
• Strong communication, listening, and documentation skills with ability to explain complex information.

Responsibilities

• Conduct home and facility visits to assess psychosocial needs.
• Provide counseling and crisis intervention for patients and families.
• Coordinate community resources and advocate for patient/family preferences.
• Document services and participate in care conferences.
• Support bereavement services and ensure regulatory/ethical standards are followed.
