Job summary

UnityPoint Health’s IL Pathways program seeks a Licensed Mental Health Professional Team Lead to provide trauma-informed, community-based services and lead the Pathways Team. You will oversee clinical care and ensure accurate documentation across the program.

This full-time role runs Monday–Friday, 8:00am–4:30pm, with supervisory duties and a focus on coordinating multidisciplinary care, crisis intervention, and quality outcomes in clinical settings and the community.

Qualifications

• Master’s degree in social work, counseling or related field from an accredited program (or equivalent supervised experience).
• Illinois/Iowa licensed with relevant credential (LCSW, LPC, or LMFT) and 2+ years in community mental health.
• Successful completion of practicum/internship with 1,000 hours of direct service or equivalent supervision.

Responsibilities

• Coordinates the multi-disciplinary process for an assigned group of Mental Health Professionals and QMHPs; ensures productivity standards.
• Provides clinical supervision and guidance to team members.
• Provides data for annual performance reviews and program metrics.
• Performs diagnostic assessments and creates individualized treatment plans.
• Delivers high-quality care appropriate to age and education level.
• Responds to crises in community locations and emergency departments with assessments and interventions.

Overview

The Licensed Mental Health Professional (LMHP) Supervisory Team Lead provides care to clients utilizing the Robert Young Center (RYC) principles of Trauma Informed Care. Provides clinical leadership and direction to the Robert Young Center Pathways Program. The LMHP Supervisory Team Lead delivers comprehensive community based mental health services and maintains accurate clinical documentation.

Responsibilities

• Coordinates the multi-disciplinary process on behalf of the individual consumer for an assigned group of Mental Health Professionals (MHP), and Qualified Mental Health Professionals Ensures that all team members meet / exceed established productivity standards
• Provides clinical supervision.
• Provides quantitative information about each staff member for annual performance reviews.
• Performs diagnostic assessments and collects diagnostic information to determine an individualized treatment plan for the client.
• Delivers high quality care appropriate to the age and educational level of the client.
• Responds to crises situations for chronically mentally ill clients in community locations and the emergency department and provides emergency assessment, recommendations, and crises intervention

Qualifications

Education:

• Master’s degree in Social Work, Counseling, Marriage and Family Therapy or a related field from an accredited program; or
• An individual possessing at least a master's degree in counseling and guidance, rehabilitation counseling, social work, vocational counseling, psychology, pastoral counseling, or family therapy or related field, who has successfully completed a practicum or internship that included a minimum of 1,000 hours of supervised direct service, or who has one year of clinical experience under the supervision of a QMHP.

License(s)/Certification(s):

• Independently licensed with the Illinois and Iowa Department of Professional Regulation in one of the following - Licensed Clinical Social Worker, Licensed Clinical Professional Counselor, Licensed Marriage and Family Therapist with two years’ experience in a community mental health setting.
• Valid Illinois driver’s license.
• Crisis Prevention Institute (CPI) training within 6 months of hire.
• Obtain certification within 2 months of hire and re-certify annually according to the requirements set forth in the Illinois Department of Healthcare and Family Services related to the Illinois Medicaid Comprehensive Needs & Strengths (IM+CANS)

The salary range listed does not include other forms of compensation which may include bonuses/incentive, differential pay, or other forms of compensation or benefits that may be applicable to this role.
