Description: This position provides personal care services to clients in accordance with the Plan of Care, ensuring safety, comfort, and well-being.

Education: High School Diploma/GED required

Qualifications: One (1) year experience in home or long-term care preferred.

Reports to: Staffing Coordinator

Primary Duties:

1. Maintain a clean, safe, environment within the client's residence, including light cleaning and organization.
2. Perform personal care activities as outlined in the Plan of Care, such as assisting with hygiene, ambulation, eating, dressing, and transfers.
3. Provide nutritional support and report any changes in the client's condition.
4. Assist with devices for daily living, prescribed exercises, and household routines.
5. Monitor and record vital signs and medication self-administration.
6. Observe behavioral changes and report to the supervisor.
7. Follow infection control policies and agency procedures.

Hours: Varies based on assignment; shifts available 24/7. Hours are not guaranteed.

FLSA Status: Non-exempt, hourly
