Job summary

Mercy Health in Lima, OH is seeking a Licensed Independent Social Worker (LISW) for the Lima Medical Office Building. The LISW provides advanced clinical social work services across the care continuum, including assessment, diagnosis, treatment planning and psychotherapy within an interdisciplinary team.

This role requires an MSW, state licensure, and at least 2 years in social services. You will coordinate care, document encounters, and support patients and families with resources and safety

Qualifications

• Master's degree in Social Work (MSW) is required.
• Licensed Independent Social Worker (LISW) or Licensed Clinical Social Worker (LCSW) as required by state.
• 2 years in social or human services setting with direct practice and treatment planning.

Responsibilities

• Conduct comprehensive psychosocial assessments and develop treatment plans.
• Provide evidence-based individual, group, and family therapy.
• Deliver crisis intervention, risk assessments, and safety planning.
