Birchwood Senior Living and Woodbury Senior Living, Lifespark Skilled Nursing Community, is looking for dedicated and passionate MDS Coordinator to join our growing team.
Schedule Available: Full Time
Location: Would float between Birchwood Senior Living (Forest Lake) and Woodbury Senior Living (Woodbury)
Wage: $44-$47 based on experience
**Sign on bonus: $2,500

In this position you will be responsible for overseeing and facilitating the successful scheduling, completion, and submission of all necessary documentation (MDS) to ensure compliance and positive resident outcomes. The MDS Coordinator will monitor coding and documentation to ensure accuracy and conducts and coordinates ongoing resident assessments necessary for the accurate completion of the Minimum Data Set (MDS), Care Area Assessments (CAA), and the resident's "Plan of Care".
