Job Title: MDS Coordinator

Birchwood Senior Living and Woodbury Senior Living, Lifespark Skilled Nursing Community, are seeking dedicated and passionate MDS Coordinators to join our growing team.

Schedule: Full Time

Location: Will float between Birchwood Senior Living (Forest Lake) and Woodbury Senior Living (Woodbury)

Wage: $44-$47 based on experience

Sign-on Bonus: $2,500

About the Role

Are you compassionate and passionate about enriching the lives of others? Join us to help seniors Age Magnificently! As an MDS Coordinator, you will become a vital part of our family, impacting the lives of residents in a Lifespark Skilled Nursing Community.

Your responsibilities include overseeing and facilitating the scheduling, completion, and submission of all necessary documentation (MDS) to ensure compliance and positive resident outcomes. You will monitor coding and documentation accuracy, conduct ongoing resident assessments, and coordinate the development of care plans. Key duties include:

  • Educating nursing staff on proper documentation, quality of care, compliance, assessments, and interventions, including POC training for nursing assistants and PCC clinical assessments.
  • Ensuring timely and accurate completion of the RAI process, including MDS, CAAs, and care plans.
  • Collaborating with the Interdisciplinary Care Team to develop comprehensive resident assessments and care plans.
  • Verifying documentation accuracy and compliance with State and Federal reimbursement guidelines.
  • Determining Medicare/Managed Care eligibility for admissions and readmissions.
  • Coordinating Medicare/Managed Care meetings and ensuring continued coverage.
  • Reviewing coverage status and managing appeals for Medicare A/Managed Care residents.
Qualifications
  • Valid state Registered Nursing licensure.
  • Minimum of one year experience in long-term care, with a strong background in MDS and regulatory compliance preferred.
