Responsibilities

• Instructs and supports individuals to develop life skills for independence.
• Collaborates with individuals, family members, advocates, care managers, and service providers to determine service strategies.
• Assess individuals’ skill levels and identify methods to teach and maintain life skills.
• Communicates with treatment team and plans/services to meet needs.
• Implements safeguards to ensure health and welfare in programs.
• Monitors progress and updates habilitation plans to prevent regression.
• Schedules, transports, and assists individuals in community participation.
