Job summary

North Jersey Friendship House, Inc. is seeking a dedicated Direct Support Professional to assist individuals with Autism and other developmental disabilities in the community and classroom-like settings, helping develop life skills and social participation.

The role involves leading groups, coordinating activities, transporting members with a 12-passenger van, documenting progress, and collaborating with the Program Manager to set new goals.

Qualifications

• High school diploma required.
• Minimum of one year experience preferred.
• Valid driver’s license with at least 3 years driving experience.
• Experience using electronic health records (EHR) preferred.
• Minimum 21 years of age.

Responsibilities

• Prepare and lead groups to support members' personal, social, vocational and independent growth.
• Assist members with following their daily activity schedule.
• Develop and maintain relationships with community orgs and employers for community inclusion.
• Plan recreational activities and daily living outings with the program manager.
• Drive up to a 12-passenger van to transport members to activities.
• Supervise members during community activities and ensure safety.
• Attend and participate in supervision sessions with the program manager.
• Document progress toward ISP goals and objectives per DDD and Medicaid guidelines.
• Review members' progress with the Program Manager and set new goals and objectives.
• Other duties as assigned by the Program Manager or Director of Day and Community Services

Job description

Hours: Monday to Friday (8:30am - 4:00pm; 37.5 hours a week)

Pay: $19.00

Primary Function: Responsible for supporting individuals with Autism and other Developmental Disabilities in the development of skills needed for greater independent functioning and community integration. Support will be provided to the individuals in the community, in social and vocational activities as well as classroom-type settings.

Performance Responsibilities: Although each position has its own unique duties and responsibilities, the following listing applies to every employee of North Jersey Friendship House, Inc.:

• Works towards the accomplishment of department/Agency goals
• Maintains confidentiality and immediately reports to management breaches in confidentiality
• Practices job requirements on an accurate, timely and regular basis
• Observes and maintains appropriate professional boundaries with members, co-workers, and supervisors
• Attend all scheduled meetings, as assigned
• Meets relevant continuing educational/training requirement
• Completes all required documentation/paperwork accurately and within established time frames
• Completes all assigned College of Direct Support and Relias trainings, as assigned

Essential Functions Include:

• Document program member's progress towards ISP goals and objectives according to DDD and Medicaid guidelines
• Work with Program Manager on review of members' progress and recommendations for new goals and objectives
• Other duties as assigned by Program Manager or Director of Day and Community Services

Requirements:
