Job summary

Youth Homes Incorporated seeks a Life Skills Coach to empower youth and young adults by building life skills, confidence, and connections to education, employment, housing, wellness, and social opportunities. You will deliver community-based behavioral health services in Contra Costa County, collaborating with clinicians and partners to support independent living goals.

Qualifications

Experience delivering trauma-informed, strengths-based coaching.
Ability to build authentic relationships with youth and families.
Familiarity with Medi-Cal, HIPAA and care coordination processes.

Responsibilities

Provide trauma-informed, strengths-based coaching to youth and young adults.
Develop partnerships with schools, employers and community organizations.
Collaborate with clinicians, caregivers, educators and partners for coordinated care.
Facilitate skill-building workshops and community experiences.
Maintain timely, high-quality documentation and compliance.
