Qualifications

• Minimum HS diploma or GED.
• Two years of experience with individuals with developmental disabilities; education may substitute.
• Valid driver’s license and ability to be insured; may need to travel using agency or personal vehicle.
• Excellent interpersonal skills required.
• Must obtain CPR, First Aid, PMT and DDS State Medication Administration certifications.
• Must be able to lift up to 75 pounds and assist in physical interventions.

Responsibilities

• Monitor consumer behavior and manage related programs; document activities.
• Document shift activities and incidents; log in Therap.
• Provide programmatic and statistical reports as required.
• Maintain safety, cleanliness and sanitation of the home and equipment; report problems.
• Maintain professional conduct with coworkers, management and outside agencies.
• Follow agency policies, procedures and licensing regulations; report violations promptly.
• Complete incident reports for consumer issues or workers' compensation.
• Implement and adhere to all commitments to consumers and agencies.
• Attend staff meetings and provide required trainings.
• Distribute medications per schedule and DDS guidelines.
• Assist with errands, meals, transport to activities and community events.
• Follow emergency procedures and notify supervisor in emergencies.
• Support normalization and person-centered planning with guardians and families.
• Help individuals develop independent living skills and daily living tasks.
• Manage assigned caseload; complete required paperwork per schedule.
• Adhere to own work schedule; field position.
• Follow rights violations reporting processes to Executive Director.
