Job summary

Acacia Health Center at Sagewood in Phoenix is seeking an Activities Coordinator for our Skilled Nursing Facility to plan and implement a varied activity program that engages residents and supports families and staff.

This full-time role requires coordinating calendars, resident assessments, and program delivery, with duties including volunteer training, purchasing supplies, and complying with state and federal regulations; CMS 5-Star rated facility.

Qualifications

• NCCAP certification preferred.
• Must be comfortable working a rotating holiday schedule.
• Must hold CPR, BLS, First Aid certifications and be trained in AED procedures.
• Must communicate effectively in English with residents and understand care plans.
• Education or experience with recreational activities or event planning preferred.
• Physical fitness to walk, stand, lift and meet physical demands.
• Patience and compassion with residents and families.

Responsibilities

• Coordinate and provide recreational and educational activities for residents in Skilled Nursing.
• Assist with monthly calendars and promotional material.
• Produce and review resident assessments.
• Suggest programs to relatives for discharge planning and refer to community resources.
• Coordinate Resident Council and family meetings; arrange special meetings.
• Direct movement of residents to recreation areas.
• Prepare reports and schedules per state and federal regulations.
• Purchase supplies; inspect equipment for maintenance and cleanliness.
• Communicate goals of the program to staff, volunteers, relatives and the public.
• Assist with directing volunteers and provide training.
• Maintain documentation of residents' progress and responses.
