Elara Caring is seeking a Licensed Vocational / Practical Nurse to join our team and help deliver high-quality home-based care. You will follow patient care plans, monitor progress, and coordinate with the care team to support positive patient outcomes.

Applicants should have an active LVN/LPN license, an accredited nursing education, and at least 1 year of clinical experience. The role involves lifting and transporting equipment and may require travel between patient homes.

Qualifications

• 1 year of experience in a clinical care setting.
• Current State LVN/LPN license.
• Graduate from an accredited nursing program (Certificate/Diploma/Associate).

Responsabilités

• Follow each patient's care plan and physician orders.
• Monitor patient progress and report changes to the care team.
• Educate patients and families on self-care techniques.
• Collaborate with the RN Case Manager to update care plans.
• Document medications and update profiles per guidelines.
• Communicate with physicians, patients, families, and team for coordinated care.
• Support care to reduce hospitalizations and ER visits.

Job Description:

As a Licensed Vocational / Practical Nurse you'll contribute to our success in the following ways:

• Follow each patient's individualized care plan and physician orders.

• Monitor patient progress and report changes or safety concerns to the care team.

• Help patients and caregivers learn self-care techniques to support their health.

• Work with the RN Case Manager to update care plans as patient needs change.

• Document medication regimens and update medication profiles according to agency guidelines.

• Communicate with physicians, patients, families, and the care team to ensure coordinated care.

• Help prevent avoidable hospitalizations and emergency room visits through proactive patient care.

• Deliver compassionate, high-quality care that supports positive patient outcomes.

What is Required?

• Current State License as a Licensed Vocational / Practical Nurse

• Graduate of an accredited Certificate, Diploma, or Associate School of Nursing

• 1 year of experience in a clinical care setting

• Ability to frequently lift, push, pull, and support up to 50 pounds, including positioning or transferring patients and moving equipment

• Must possess a valid driver's license, maintain an acceptable driving record, OR have reliable transportation to travel between patient homes and assigned work locations.

• Experience in a hospice or home health environment is preferred
