Job summary

Riverside PostAcute Care is seeking a self-motivated Licensed Vocational Nurse (LVN) to provide direct patient care in a skilled nursing setting. The role includes nursing assessments, care planning, and supervising unlicensed staff to ensure high-quality patient outcomes.

You will work with a team to coordinate care, maintain patient safety, and participate in mandated in-service programs while promoting confidentiality and professionalism.

Qualifications

• Current LVN license in CA.
• Maintain CPR certification.
• Demonstrates observation, problem solving, verbal and written communication.

Responsibilities

• Perform patient evaluation and direct patient care.
• Monitor patient care provided by unlicensed staff.
• Assist physician in rounds and examinations as needed.
• Document care in EMR according to procedures.
• Maintain PHI confidentiality at all times.
• Ensure effective shift-to-shift communication among nursing staff.
• Collaborate with other departments to meet patient needs.
• Promote a culture of safety and ongoing learning.
