Rialto Post Acute Center is seeking a licensed vocational nurse (LVN) to provide care and treatment under physician orders. You will work in a team environment and operate within the State-defined scope of practice, reporting to physicians and registered nurses.

The role includes patient assessments, administering medications, care planning, supervising CNAs, coordinating with physicians, documenting care, educating patients and families, and completing paperwork in line with company policy and

Qualificações

• Graduate of an accredited school of Nursing.
• Valid State licensure as LVN.
• Current CPR certification.

Responsabilidades

• Assesses patients by physical examination and pertinent tests.
• Administers medications and treatments as ordered.
• Participates in care planning and coordinates with the team.
• Supervises LVNs and certified nursing assistants.
• Communicates changes in patient condition with physicians.
• Documents assessments and care per policy.
• Educates patients and families on health issues.
• Completes required forms in accordance with regulations.
