Program Description
Serving adults and transitional age youth with serious mental illness, the Adult Full-Service Partnership (FSP), TAY FSP, TAY Acute Linkage, and Adult Intensive Case Management programs provide an extensive array of recovery-oriented, age-appropriate services and supports, including linkage to housing and basic needs assistance, physical health care, benefits assistance, employment services, and integrated mental health and substance abuse treatment services. Every attempt is made to ensure continuity of care and to develop a community support system. Individuals are connected to appropriate resources and community health and mental health services, facilitating the development and implementation of their plans to achieve their desired outcomes. The programs follow an assertive community treatment model, providing medication and other wraparound services with case management for those with chronic serious mental illness. This continuum of services provides outpatient mental health treatment to 16–60-year-olds living with severe mental illness. The programs consist of multi-disciplinary teams consisting of a psychiatrist, two psychiatric nurse practitioners, two LVNs/LPTs, a program director, a clinical supervisor, clinical case managers, case managers, and an administrative manager. Services are provided to clients both in the office and in the field.

Job Description
The Licensed Vocational Nurse/Licensed Psychiatric Technician exercises medical responsibilities for psychiatric evaluation and health assessment, medication administration including monitoring for side effects, monitoring for changes in health and mental health status, coordinating care with prescribers, making referrals for additional care, and pharmacy coordination and communication, as part of the treatment team. The LVN/LPT assists with crisis interventions, including 5150 evaluations, as needed, and communicates medical issues with Primary Health Care providers. The LVN/LPT maintains client caseload and clinical records in accordance with mandated standards, consults and provides on-going education to non-medical staff in psychopharmacology and medical aspects of patient care. Additional duties as assigned. This position is based out of our San Francisco clinic and will be working with both TAY and Adult mental health divisions. The provider’s schedule will include on-site as well as outreach care, as needed.

Specific Responsibilities

  • Exercises medical responsibilities for psychiatric evaluation, medication assessment, and medication monitoring
  • Works with an interdisciplinary team that includes clinical case managers, peer/community specialists, and other allied health professionals in the community
  • Works in collaboration with the clinical team to provide therapeutic support and psychoeducation to clients, including but not limited to harm reduction, relapse prevention, and motivational interviewing
  • Works in collaboration with the clinical team to make appropriate referrals and linkages to other services in support of clients.
  • Coordination of care with outside providers to provide more comprehensive care
  • Maintains clinical records in accordance with mandated standards, including conducting psychiatric diagnostic evaluations.
  • Consults and provides on-going education to non-medical staff in psychopharmacology and medical aspects of patient care
  • Performs additional duties as assigned by supervisor or assignee.

Minimum Requirements and Qualifications

  • Completion of training in an accredited Licensed Vocational Nurse Program or Licensed Psychiatric Technician program
  • Active Licensed Vocational Nurse or Psychiatric Technician license from the California Board of Licensed Vocational Nurse.
  • Experience working with SMI (severely mentally ill) populations and community mental health settings preferred.
  • Demonstrated commitment to the welfare of individuals and families with mental and behavioral health issues
  • Knowledge of the information and techniques needed to diagnose and treat human injuries, diseases, and psychosis. This includes symptoms, treatment alternatives, drug properties and interactions, and preventative healthcare measures.
  • Complex problem solving, critical thinking, and social perceptiveness skills, i.e. being aware of others' reactions and understanding why they react as they do.
  • Service orientation skills; actively looking for ways to help people.
  • Demonstrated interest in ongoing learning and training.
  • Ability to be a team player with a welcoming attitude.
  • Willingness to travel for outreach to clients in the community.
