Qualifications:

  • Active, unencumbered CA LVN license
  • Current CPR certification (hands-on component)
  • TB skin test - 2 step (within last 12 months)
  • At least one year of hands-on nursing experience
  • Reliable transportation
  • Proficiency in clinical assessments, documentation, and compliance with policies and procedures
  • Continuing education as required by state

Position Overview:

The LVN provides and documents skilled nursing care under RN supervision, following care plans and physician orders, while maintaining confidentiality and professional boundaries.

Essential Job Functions:

  • Deliver and coordinate quality patient care according to physician orders
  • Assess patient condition continuously and report changes
  • Document all activities and assessments promptly
  • Participate in updating nursing care plans
  • Implement care based on assessments to meet expected outcomes
  • Recognize and respond to changes in patient needs
  • Comply with policies, regulations, and the Nurse Practice Act
  • Use infection control measures per OSHA standards
  • Educate patients and families on disease management, self-care, and prevention
  • Engage in ongoing professional development

Requirements:

  • Graduate from an accredited nursing school
  • Current, unrestricted state nurse license
  • Current CPR certification
  • Proficiency in assessments, documentation, and policies

Additional State-Specific Requirements:

  • SC: 1 year pediatrics experience
  • CA: 1 year of experience under current license
  • LA: 1 year of experience as a licensed nurse

Preferences:

  • 6 months recent clinical nursing experience
  • Home health experience

  • Good organization and communication skills

Physical Requirements:

  • English proficiency
  • Ability to travel and lift 50 pounds
  • Ability to reposition patients and move equipment
  • Prolonged walking, standing, bending, kneeling, reaching, twisting
  • Visual and hearing acuity, strong sense of smell and touch
  • Respond appropriately to emergencies

Environment:

  • Function in diverse environments, possibly exposed to allergens and infectious materials
