VitalCaring Group is seeking a compassionate LPN/LVN to provide direct, end-of-life care in a home health setting in Texas. You will implement care plans, manage pain and symptoms, and support patients and families with dignity and comfort.

Qualifications

• Active LPN/LVN license in the state of practice (Texas).
• Current CPR certification required.
• 1–3+ years of nursing experience preferred (hospice, home health, or med-surg).
• Strong clinical judgment, communication skills, and emotional intelligence.
• Valid driver’s license, reliable transportation, and auto insurance.

Responsibilities

• Provide direct patient care in accordance with the Nurse Practice Act, with a focus on end-of-life support.
• Implement individualized plans of care under supervision of a Registered Nurse.
• Deliver effective pain and symptom management, monitoring and adjusting based on patient response.
• Support patients with daily care needs while ensuring comfort, dignity, and safety.
• Educate patients and families on disease progression, medications, and comfort measures.
• Document care accurately and timely in the EMR and communicate changes to the care team.

What You’ll Do

• Provide direct patient care in accordance with the Nurse Practice Act, with a focus on end-of-life support

What You Bring
