Job summary

VitalCaring is seeking a Hospice Licensed Nurse (LPN/LVN) in Mississippi to provide compassionate end-of-life care with dignity and comfort. You will work with an experienced clinical team, deliver patient-centered care, and educate families on care plans.

This role offers autonomy with support, collaboration across RNs, aides, social workers, and chaplains, and a benefits package including medical, dental, vision, 401(k) match, PTO, and more.

Qualifications

• Active LPN/LVN license in the state of practice.
• Current CPR certification.
• 1-3+ years of nursing experience preferred (hospice, home health, or med-surg).

Responsibilities

• Provide direct patient care with end-of-life focus.
• Implement individualized plans of care under RN supervision.
• Deliver pain and symptom management and adjust as needed.
• Educate patients and families on disease progression, medications, and comfort measures.
• Document care accurately and timely in the EMR and coordinate with the care team.

What You’ll Do

• Provide direct patient care in accordance with the Nurse Practice Act, with a focus on end-of-life support
• Implement individualized plans of care under the supervision of a Registered Nurse
• Deliver effective pain and symptom management, monitoring and adjusting based on patient response
• Support patients with daily care needs while ensuring comfort, dignity, and safety
• Educate patients and families on disease progression, medications, and comfort measures

Coordinate & Communicate

• Document care accurately and timely in the EMR
• Communicate changes in patient condition to the RN and care team
• Collaborate with interdisciplinary partners (RN, HHA, Social Work, Chaplain, Therapy)
• Participate in care planning and coordination to ensure holistic support

• Follow best practices and clinical standards in hospice care
• Maintain proper use of equipment and adhere to infection control protocols
• Participate in quality improvement initiatives and team development
• Continue professional growth through training and education

What You Bring

• Active LPN/LVN license in the state of practice
• Current CPR certification
• 1-3+ years of nursing experience preferred (hospice, home health, or med-surg)
• Strong clinical judgment, communication skills, and emotional intelligence
• A genuine passion for patient-centered, end-of-life care
• Valid driver’s license, reliable transportation, and auto insurance
